CEO ousted in shake-up at hospital


Move at DECH in Machias follows federal sanctions

MACHIAS, Maine — Wayne Dodwell, president and CEO of Down East Community Hospital since 2002, was placed on administrative leave Friday, just hours after federal authorities notified the hospital it would be cutting off Medicaid reimbursements in less than two weeks.

In a terse, four-sentence statement Monday, DECH announced that “experienced hospital administrator Craig Jesiolowski has been appointed interim chief executive officer.”

Jesiolowski is a regional vice president with Quorum Health Resources, a national firm that provides management services to numerous hospitals, including DECH.

Calls to Jesiolowski had not been returned by Monday evening.

The internal shake-up comes on the heels of an announcement last Friday that the Centers for Medicare and Medicaid Services, the federal licensing authority for hospitals, informed DECH that as of July 10 the hospital will no longer be able to accept Medicare reimbursements. A state takeover is being considered.

Jesiolowski will serve as administrator of the hospital while discussions continue with the Maine Department of Health and Human Services and the CMMS regarding the hospital’s future operations, according to Julie Hixson of DECH’s public relations office.

“As those discussions progress, DECH continues to provide both scheduled and emergent inpatient and outpatient care,” Hixson said in a phone interview Monday.

She said she did not know the process by which Dodson was placed on administrative leave.

No further information was provided by the hospital Monday.

In a statement last week, Quorum indicated that its suggestions for better management had not been followed by the hospital’s board of trustees.

“Over the last 18 months, the DECH board has developed a clear preference in how they desire to manage the hospital, particularly in relation to regulatory compliance,” spokeswoman Susan Hassell said in the statement. “In some instances, their preference differs from our company’s approach to hospital management. More recently, the board has made numerous decisions inconsistent with [Quorum] recommendations.”

Hassell said the DECH board refused to allow Quorum consultants to be involved in federal and state surveys and rejected Quorum’s proposals for a change in hospital leadership.

Dodwell has been a lightning rod for criticism by local groups that have called for his resignation for more than a year, pointing to mismanagement and poor leadership as the reasons the hospital has come under state and federal scrutiny.

Annie Dickinson, one of the principals in the Committee to Save Our Hospital, said Monday she wasn’t surprised at the recent hospital situation.

“We felt it was coming,” she said. “We never had any problems with the staff — the doctors, nurses or other professionals — just with the way the hospital was managed. It was purely a management issue.”

Dickinson said she hopes the state puts DECH into receivership. “We will then be able to get back to business and treat our community,” she said.

Janet O’Neal, a member of the Committee to Save Our Hospital, said removing Dodson was just a beginning.

“We have no confidence in the board [of trustees],” she said Monday. “The board is the boss, not Dodwell. We wanted him and the board to go.”

Attempts on Monday to reach Walter Plaut, chairman of the DECH board of trustees, were unsuccessful.

Just one month ago, at the hospital’s annual meeting, Dodwell reported the hospital experienced another strong year on the quality, patient satisfaction and financial fronts, despite some challenging times.

“In 2008, we experienced some of life’s best and most difficult moments,” Dodwell said at the meeting, according to a press release from the hospital. “We are an organization of human beings and just when we believe we are operating at our very best, life reminds us that we can always do better. Despite these difficult moments, our hospital continued to perform well in regional and national quality, patient satisfaction and financial indicators.”

Dodwell reported that during 2008, Down East Community Hospital earned two blue ribbons from the Maine Health Management Coalition for its patient safety and quality of care initiatives, and the hospital’s National Quality Alliance scores outperformed the majority of Maine and U.S. hospitals.

Also, the hospital’s patient satisfaction scores rose to the top quartile for in-patient and emergency department services, according to Press Ganey, an independent company that measures patient satisfaction for more than 1,000 hospitals nationwide.

At the same time, the hospital showed a positive operating margin for the fourth consecutive year. “While approximately one-third of Maine hospitals lost money from operations in 2007 and 2008, Down East Community Hospital continued its strong financial performance,” Dodwell said.

But that optimistic view didn’t help when it came to dealing with federal and state regulations. Last April, CMMS informed the hospital that it was prepared to terminate DECH’s participation in the Medicare program.

The sanction came on the heels of a recent investigation that revealed numerous failures in emergency room procedures at the Machias hospital after an incident last fall.

It was the third federal survey in about 15 months that found DECH out of compliance and required drastic changes in various procedures at the hospital.

Down East Community Hospital has been under close federal scrutiny for more than a year, after the high-profile death of Reid Emery in January 2008. Emery of Eastport was a DECH patient who checked out of the hospital against doctors’ wishes on a cold, snowy evening and, heavily drugged from his stay at the hospital, was found dead the next day in a nearby snowbank.

The Maine Department of Health and Human Services also has kept a close eye on concerns at the hospital recently, and in February the state demoted DECH to a conditional license.

Cheers!!! now clean house on "certain" other managers at DECH and that place might just stand a chance!!! Perhaps mandating that all supervisors take a reading and writing test and if a certain supervisor in there can't pass it give her the boot as well! That place needs professional EDUCATED leadership in order for that place to run and people who know how to manage (and I might add know the difference between there & their and to, too, and two when sending out emails to their staff)

Too bad for DECH. Quorum was a division of Columbia Healthcare, and split off weeks before the latter was targetted in a massive fraud investigation by the feds. They are not a fave of mine. Anyway, what the hell does "heavily drugged" mean? Lipitor? Viagra? Aspirin? If someone walked out of a bar heavily intoxicated after they were advised to call a cab, is the bartender responsible for what happens later? Under the law, you can't prevent someone from leaving any place unless you have probable cause to believe that they are a danger to themselves or others. He was advised to stay, but instead he walked out under his own power. The hospital is on the street, not on an island or at the top of a cliff. I'd rather hear the facts than supposition.

You should AT LEAST call the cops and tell them someone is a danger to themselves, not say as one did, 'none of my business'. He left in pajamas, sliippers and awindbreaker.

Makes me feel all warm and fuzzy knowing ppl are in good hands at DECH! NOT!! Even common sense would tell you to call the cops!! An elderly man, heavily medicated, snowstorm, improper clothing and obviously not in the right state of mind....and the personnel/staff didn't see anything WRONG with this? HELLO...that's just a lawsuit screaming to be filed!...now if this were a member of THEIR family....you know what I'm getting at.

I believe in some forum I read that one member of the public relations department was making in the neighborhood of $140,000. Another name is referenced as speaking for the public relations department. How many people in public relations are needed for a 25 bed critical access facility? How much does it cost to operate?

Lets look at Mr. Dowell;s statement: ". . ".life reminds us that we can always do better. " Interesting choice of words, since it is apparent that is a reference to Reid Emery.

Just asking.

HOW MANY hospitals BELONG to the Maine Health Management Coalition? FOURTEEN, excluding facilities joined to a parent entity. That was UP from TEN in 2002. FOUR more have joined in SEVEN years. They must be doing something right. . .

THIRTY THOUSAND according to WikiAnswers. So,how many survey agencies did DECH have to contact to find the one agency that only interviews THREE percent? HOW MUCH DID THAT SURVEY COST? Better still, WHAT ELSE DO THEY SELL? A visit to their website and how all that works was very enlightening, especially when you consider again, DECH is a 25 bed Critical Access Facility.

And Walter Plaut, the board chairman, still has no comment. Hopefully this dunce follows the CEO out the door. This guy couldn't run a hot dog cart. He is totally inexperienced in business and it shows in the way the hospital has been mismanaged.

The tragedy here is that the incompetence is not just Dodwell and his girlfriend in management. The incompetence is with the medical staff too. A group of loser rejects that can't get a medical position at any other hospital in Maine. Fire the whole staff and start all over, or just shut DECH down. The whole medical staff has to be rebuilt from the CNA's on up. Perfect example was when DECH hired an Illegal Alien Canadian male nurse. This was bad enough, but when he turned out to be an addicted junkie, this was too much. Is this the kind of medical staff Washington County wants?

I think one manager in particular should be tossed... this supervisor from what I understand is still so high on her horse that all she does is run around for the most stupid reasons claiming that she is the supervisor every five seconds to anyone that will listen to her! Its pretty bad when you have her in your face telling you that you should of gone through her first in order for the next toilet paper roll to be changed! Maybe this supervisor should worry more about learing how to spell and send out memos and emails correctly lmao didn't she just take a microsoft word class? "oh wait thats right alll she did was copy homework ect from other staff memebers who was taking that same class"

Dodwell has been President and CEO since 2002. Craig Jesiolowski has been appointed interim CEO. He is also the Vice President of Quorum. Anyone can check the following fact online: On April 23, 2001 Quorum Health Group agreed to pay $85 million to settle claims that it defrauded the Medicare program from 1985 to 1999, the U.S. Justice Department announcement. My question is: Did the Board not check into the qualifications/record of Quorum Health Group prior to having them provide Dodwell as President and CEO one year after the Justice Department settled their claim with Quorum? Anyone employed by a corporation knows that there are annual performance reviews. Therefore, it's my assumption that Quorum, as well as the Board of Directors, need to take responsibility for their failure/disregard in maintaining Dodwell in his position. And now to have Mr. Jesiolowski acting as interim CEO amazes me. Having had my own experiences at DECH, I strongly recommend anyone with health problems go somewhere else for their care...even if that means having to drive further than one would care to.

This ongoing situation is bad enough without having the ousted administrator variously referred to by 2 different names, neither of which is his correct name...Dodwell, Dodson...neither is his name.

ODD: Because then even DECH's home Web page has mispelled his name. They have it listed as "Dodwell."

Obviously whoever is sending out emails is not using spell check!! I too had someone at a former job at RMCL that did not use correct grammar and spelling when sending out memos. Whoever this supervisor/manager is needs to be told that the memos/emails being sent out are not correct. Before any more judgements about Mr Emery's demise I believe there should be a total check of accuracy. I do believe his family was called and they did not want to come out in the weather to pick him up. True things could have been handled better, I held a position where I was responsible for preparing pt's discharge inst after the Dr had written them. On occasion a pt would be anxious to leave, the Dr was called, necessary paperwork signed which is the pt is responsible for themselves and that they are leaving against physicians advice . No one has ever mentioned if this piece of paper was signed and witnessed. I feel bad for the Emery family because every time DECH is mentioned that situation is brought up over and over. As for Mr Dodwells release it will be interesting to know if he is still being paid his high salary. Will the board be re-organized as well? I imagine a lot of staff are worried that they to will be released, which may be worthwhile. I do not want to see this hosp close as it is needed in this area. I will say some registration staff needs to be spruced up, in dress and manners.

If you approached this supervisor and told her anything like that she would turn it around and use it against you or she would be emailing lies to Lynnette about it so it would be a no win situation * people caught on to her crap after awhile and found out how to get in her email (it wasn't hard not only can she not spell she didn't have any brians when it came to choosing her password either)

If DECH still has the person or persons that allowed that unfortunate man to walk out in the middle of a raging snow storm in slippers, pajama bottoms and a windbreaker, the facility needs to be shut down. I don't care who they are, or who they think they are. That place needs to be shut down if they allow these people to continue to be employed there.

Never been to DECH, probably never will go there, either. But, I have been to several other Maine hospitals and NONE compare to the quality of care you get in Boston. Until our hospitals get off their high horse and acknowledge that there are best practices, well-documented, that should be followed in patient care, we will continue to get sub-standard care at a very, very high price. I have found most doctors and administrators in Maine to be very ego-centric and they think they have all the answers. One visit to a Boston hospital and you will realize we are playing in the little league of hospitals here in Maine.

The irony is costs would go down and quality would go up if they would just take the time to see what others do instead of insisting they "know better" and don't need to. Very sad for all of us to be trapped in the health care system we have in Maine.

It might be interesting to note that vmrider1 is an EX employee of DECH, and rightfully so, apparently. Hacking into one's supervisor's email account doesn't seem to be an issue to brag about, and more than likely would have been grounds for immediate dismissal had she and her cohorts been caught. Shame on you vmrider - and good riddance.

Yikes ... how unprofessional many of you sound ... if things are/were THAT bad then LEAVE ... I can't help but believe that your attitudes toward administration had to have carried over to the patients you were there to help ... I'm glad I was never one!

I would suggest that BDN stop the verification of email address as a requisite for postings. It is useless. As there are various email servers , possibly vmrider1,karmababykarma, and jonesportdevil may represent one individual not a group attacking DECH.

If hacking into a hospital's intranet/internet account is not illlegal it certainly is unethical. I for one would want the hospital to check into this to ascertain just what information was gleaned and if this action violated HIPPA laws.
