Shortage worsens as H1N1 continues to spread in state

Fewer than two-thirds of the very highest-priority Mainers — children and pregnant women — have been vaccinated against the H1N1 flu, largely due to the national shortage of vaccine, according to Maine’s top public health official.

“I know there are a lot of people in Maine who want this vaccine, and we want to get it to them,” said Dr. Dora Anne Mills, director of the Maine Center for Disease Control and Prevention. But the state can’t distribute doses it doesn’t have, she said.

Meanwhile, as the shortage worsens, H1N1 continues to spread in Maine. Mills said the outbreak is unlikely to peak here for at least another two weeks. This past week, 50 people were hospitalized with severe illness, half of them children. Since August, five adults with underlying health problems have died of complications of the H1N1 virus. Flulike illness now accounts for one of every six visits to Maine emergency departments.

Mills said Thursday she continues to press federal officials for more doses of H1N1 vaccine for Maine. In the meantime, she said, Mainers in high-risk categories, including youngsters, expectant mothers, and adults with underlying health problems, should ask their doctors for anti-viral medications such as Tamiflu if they develop flulike symptoms — high fever, sore throat, cough and extreme fatigue.

As of the end of this week, Mills said, Maine will have received only 210,500 doses of the new vaccine. That falls far short the 527,000 doses the U.S. Centers for Disease Control and Prevention promised in August to ship to Maine by the end of November. In a revised statement in September, the federal agency said approximately 321,000 doses would be in Maine by now.

There are about 300,000 children and pregnant women in Maine, Mills said — the most important groups to protect, from a public health perspective.

“This is a very frustrating situation for all of us, as we thought we would have much more vaccine by now,” Mills said. Of the doses that have been received, about 12,000 were administered early on to another high-priority group, health care providers who work in emergency departments and other settings with a lot of public contact. The primary goal of vaccinating that group is to prevent them from infecting members of the public, Mills said.

On a brighter note, Mills said, nearly 95 percent of all schools in Maine have now held H1N1 vaccine clinics for their students, although only about 60 percent of Maine students have actually received the vaccine.

More than 140 schools have reported high absentee rates, at least in part due to H1N1. Two schools, including Washington Academy in East Machias, have experienced such high rates of illness that they have shut down for a few days.

“We decided the prudent thing was to give everyone a chance to rest and recover,” said Judson McBrine, headmaster at Washington Academy. Of the 415-member student body, 115 were sick this week, along with at least 11 faculty and staff, he said. The school canceled classes on Wednesday and Thursday, along with all sports and other after-school activities.

Washington Academy offered all students a seasonal flu vaccine earlier in the year, and will have an H1N1 clinic today. McBrine said the school received about 200 doses of the H1N1 vaccine earlier this week. On Monday, U.S. Sen. Susan Collins, R-Maine, teamed up with Sen. Joe Lieberman, of Connecticut, to demand answers from the federal Department of Health and Human Services about the national shortage of H1N1 vaccine and the distribution of available supplies. The Committee on Homeland Security and Governmental Affairs, of which Lieberman is the chairman and Collins is the ranking member, conducted hearings on the matter this week.

On Thursday, Collins and Lieberman followed up with a second letter to Health and Human Services Secretary Kathleen Sebelius, urging a redesign of vaccine distribution strategies so that states like Maine that are still experiencing a rapid increase in cases get vaccine sooner than states where the outbreak has leveled off.

In her remarks on Thursday, Mills said that even if vaccine doses in Maine don’t become readily available until after the outbreak peaks, people still should get vaccinated when they can.

“We expect H1N1 to continue to circulate for a long period of time, maybe two or three years,” she said. Gov. John Baldacci on Thursday renewed his H1N1 civil emergency proclamation, originally signed on Sept 30. The designation facilitates the distribution of vaccines and anti-viral medications and activates a multiagency response plan.

From other reports I've seen, including an AP article recently, the manufacturers had only from April until Sept. to make and satart distributing the H1N1 vaccine. Yeah, they were overly optomisitc on how much they could deliver and when, but that's hardly the fault of this or any administration. As for distribution, the CDC ships the vaccine to the states and internal distribution is up to the state agenices. On a National scale, in comparison to other states, Maine rates very well in distribution of the vaccine it has received. Horror stories abound elsewhere, especially in CA where counties with 7 times the population of smaller counties recieved only the same (and insufficient) amount of vaccine as the smaller counties. Go figure. Bottom line: it may be convenient for those who dislike Obama and the administration (note, no "H" word on my part anyway) to continue to blame him (and them) for EVERY detail they're dissatisfied with, it isn't always correct to do so.

First if all, the pharmaceutical companies are overwhelmed trying to get the H1N1 2009 vaccine manufactured. Generally they don't vaccinate this many people in our country at one time. Normally, school clinics aren't held to vaccinate all children and faculty members. We rather, vaccinate those who NEED flu vaccines such as those with "underlying medical conditions" first.

Secondly, if you all think that the pharmaceutical companies are going to be able to get your seasonal flu vaccine out to you on time, you're sadly mistaken.

I do feel for those who NEED this vaccine, you're all being left in the dark.

Not getting the shot - you couldn't pay me nor force me to get it. I'm sticking with my healthy diet, Vitamin C & D, and medical marijuana.

This is disgraceful -- people have died unvaccinated, while Citicorp and Goldman Sachs enjoy timely delivery of more doses than public hospitals, universities and schools. An excellent article written by a PhD in cell biology turned writer explains the causes of the shortage. Before we surrender vaccination to "free-market" forces, however, we would do well to remember the shortage of 2004... available supply went to low-risk individuals while those who so desperately needed it went without, or, ironically, went to Canada...!

Maybe the State would have enough of the vaccine if the schools in my district would stop vaccinating the kids against their parents wishes. That happened Wedesday to one school, 10 families had their kids given the H1N1 when the parents specifically requested they not get it. And then to have the schools tell parents even after they have expressed their decisions that if their child's name is on the list then they will get vaccinated. I know some of the parents that had their kids given the shot against their wishes are upset, mad, worried and taking action.

Let's hope those parents find a darn good attorney and not some half baked POS who doesn't know what he's doing, and causes them to lose their case. If I were those parent's I'd be FURIOUS!!!

downeast4me- those are some pretty strong accusations you are making. The schools in Downeast did send home 2 separate forms for parents to sign- one for the seasonal flu vaccine and another for the H1N1 vaccine. I would first say that the parents involved need to find out where those supposed signed consent forms are. Possibly either the nurses/ parents were confused. I cannot imagine vaccines would be given to children with neither signed form.

I can't imagine it either...at my child's school district, if you were younger than 9 you had to have a parent present when you rec'd the vaccine.

Coastalblogger and Justmyopinon, I agree. It was the same with my childrens JrHS and HS...which is located in the same district.

The PA at my childs school administering the shots actually called me at home due to an error I'd made on one of the kids the form. Because of this error, they couldnt vacinate either child. To me, this error was insignificant, but to the schools RN and the PA, it was enough to not only call my home, but my husbands work cell, and my parents (who are the emergency contact) to re-verify my consent.

They wouldnt take verbal consent over the phone either. My mother had to drive to the school and correct the form...and even then, they called me a 3rd time to be sure she had my permission to do so.

Quite honestly, it sounds like rumors downeast4me. I just cannot imagine a Reg. Nurse risking her license and making such a huge error...and according to your accusations...with more than ten children.

I also cant imagine any parent (that has the ability to read and research) would choose to not vaccinate their child. The risk for illness is far greater than the side affects from this vaccine.

The more people that are vaccinated, the safer the community is.

their problem is with the egg...............part of the vaccine, and i chose not to vaccinate my kid, he just got over the swine flu got checked up today no fever ready to go back to school, i would much rather have my childs body build up its own immunity then have some un-tested crap injected in his system, your body works effieciently on its own with right fluids and vitamins to keep immune system, injecting something "foreign" into your body is not natural, screwing up the immune system and effeciency which is why they didn't prescribe my child any antibiodics......his immunity system we'll build much faster then having the shot.

And people 4 days of a cough and a fever thats it cough still lingering, its only contagious during the fever and within 24 hours of the fever subsiding, if your child has conditions get the shot if you will but also be prepared that they may not take the shot well

For only those that have the medical or weakened immunity are at risk, if your healthy deal with it and move on its alot of hype, the doctors office is treating it like a bad cold right now, now tests, these are the symptoms slap em on the but and keep the fever down.

36,000 people die from the flu each year. That is more than die in car accidents.

One has to weigh the risk of illness and death from the swine flu versus the vaccine. So far, looking at all of the facts, I have come to the conclusion that the risk of dying from swine flu is much much higher for me and my kids than dying from the vaccine. Therefore we will get it. Of course this could change, so I will keep my ears posted for any possible side effects of the vaccine. Everyone must make their own decision, just try to make it based on facts and not hyped up junk science or rumors.

I have only one question. Have the members of the US house & Senate gotten their H1n1 vaccine?

7:38 AM. highly doubtful. GW's "Katrina" was basically created and perpetuated by the Liberal Media. The only way this would be any ones Katrina, would be if the Republicans had control over the House or Senate, or a Republican was in the White House.

Makes me wonder what was really in it for the district's to administrate these clinics??? Did they get something extra for every kid that got the shot????

As we all know, reporting on the subject of H1N1 has been abysmal. Included in this assessment should be the lack of contextual reporting on the efficacy of all influenza vaccinations over time. A Dartmouth research group (comprised of physicians of varied disciplines) which studies excesses in medicine, has highlighted some important issues within the current sea of media coverage: "By choosing to highlight the annual number of flu deaths (36,000), the CDC employs an attention-grabbing tactic often used by public health and disease advocacy groups. It's a tactic of which readers should be advised if they want a clear picture of the risks they face." Fact: The U.S. flu-related death rate has not budged in 20 years, despite a significant increase in the use of vaccinations. (Cochrane Research Group). It is also a fact that it is very difficult to know how many people die from ANY given disease because there is often much uncertainty in determining the cause of death. This is particularly true for the flu, which also makes it a good candidate for statistical abuse by pharmaceutical powers. It shares symptoms with many other diseases, and people most likely to die a flu-related death are also at high risk for many other causes of death. (For context, the chance of a flu-related death is slightly lower than the chance of dying from a fall or other accident.) Studies have also measured another outcome: how vaccination affects days lost from work. On average, there are about 0.16 fewer days lost from work per person vaccinated. Another way of saying this is that about 5 percent of those vaccinated avoid missing about three days of work because of the flu. (That is, 0.16 days divided by the 5 percent who benefited from vaccination equals 3.2 days.) The other 95 percent vaccinated got no benefit. http://www.vaoutcomes.org

Press releases, like those by the CDC, frequently present data in exaggerated and unbalanced formats, while failing to mention study limitations and any conflicts of interest. Also omitted are the reports of many qualified independent researchers who have shown influenza vaccines to be highly ineffective for the general poplulation, and may actually be harmful in the long term. All we get from the CDC is that "the H1N1 vaccine is made the same way as all influenza vaccines." Again, I would rather see reporting with more editorial oversight, while including information that puts results in context with other studies.

I speak from first hand knowledge...My youngest nephew in Kindergarten was one of the kids who got the shot against parental consent.

CautionWetTaint~ Actually, I'm not on the "hype" bandwagon at all. Exactly what are you referring to? These kids go to school to learn, not to have injections. We spent school money to bus these kids to auditoriums... and how much do you think the kids actually learned that day, when their entire day was disrupted? When these kids returned to school, teachers were left to monitor them for side effects. Not their job!!

What many do not realize is the fact that most of the 36,000 who die from the flu every year are either very old and/or, have many underlying health conditions to begin with... making them prime candidates for death. These folks should always be the first to get flu shots.

Nor do people realize that as of 2009 the mandated vaccines for children from birth to the age of 18 is around 68. Then add all the flu vaccines from 18 to 65. The accumulated toxins over that period of time does damage to the body, without question.

How surprising any school nurse would administer vaccine, or anything else for that matter, without express, written permission downeast4me. Our own nurse asks for written permission for Tylenol at the beginning of each new school year, and separate forms for H1N1, and seasonal flu vaccine. What you describe is quite a deviation from what would be considered standard practice...
