Michaud tells of talk with Obama on Medicare

BANGOR, Maine — Medicare payment rates in Maine are the second-lowest in the nation, reimbursing hospitals and physicians somewhere between 20 percent and 30 percent less, on average, than the cost of providing services.

For Rep. Mike Michaud, the opportunity to change that calculus came a little over a week ago when President Obama telephoned him to talk about his support for the Affordable Health Care for America Act, the U.S. House version of the sweeping national health reform effort in Congress. It was Friday, Nov. 13, the day before the House voted on the historic legislation.

“The president made it clear he was willing to work with me and Sen. [Olympia] Snowe on the Medicare issue as it relates to Maine,” Michaud said Monday, pointing out that Maine’s senior senator has many of the same concerns he does.

One possibility they discussed was increasing Maine’s reimbursement rate to match Boston’s, a measure that would boost hospital coffers here by more than $100 million a year, Michaud said. Another idea was to somehow dismiss Maine from its obligation to repay the federal government an annual $40 million prescription drug obligation related to the high number of low-income, elderly Mainers who qualify for both Medicare and Medicaid. A third scenario was to increase Medicaid payments to Maine’s primary care providers to come in line with Medicare rates instead of the approximately 40 percent less that Medicaid pays.

Armed with the assurance of Obama’s interest in helping to solve Maine’s Medicare and Medicaid dilemmas, Michaud overcame his several reservations about the House measure and added his vote to the Democratic majority, helping to narrowly pass the bill 220-215.

Michaud was at Penobscot Community Health Care in Bangor on Monday to accept a Profiles in Courage award from the National Association of Community Health Centers. The Rev. Robert Carlson, PCHC president, lauded Michaud’s support of national health care reform.

“He had reservations, but he voted in favor of it just to keep the conversation going so we can begin to sort through all these complex issues,” Carlson said, speaking to a small group of staff members at the Union Street health center. The national organization awarded similar recognition earlier this year to Snowe and Sen. Susan Collins for their support of Obama’s American Recovery and Reinvestment Act — two of only three Republicans to support the federal stimulus package.

The central goal of health care reform legislation in both the House and the Senate is to provide at least basic coverage to all Americans, an effort Carlson said will support the expansion of federally funded community health centers like PCHC.

The centers provide comprehensive care for enrollees in Medicaid, Medicare and private insurance policies, as well as a sliding-scale fee for people with no coverage. At its sites in Bangor, Brewer and Old Town, PCHC provides dental care, mental health services and medical care to nearly 50,000 area residents — “from psy-chiatry to podiatry and everything in between,” Carlson said.

Increasing the Medicare reimbursement rate for hospitals and doctors in private practice would not raise revenues at clinics like PCHC, said CEO Ken Schmidt.

“But I think it would be great if they could go to the Boston rate,” he said. Especially in underserved rural areas like most of Maine, he said, “it might encourage more private physicians to take care of Medicare patients and do it without losing money.”

The Senate version of the health reform bill, which will be debated on the floor next week, includes a Snowe-sponsored amendment that would lift a per-visit cap on Medicare payments to allow clinics like PCHC to be paid for the full cost of delivering their services, Schmidt said.

Assuming the Senate approves its own version of health reform legislation, it will be merged with the House version for final congressional approval and signing by the president.

Michaud has said his support for the House bill does not ensure he will vote for the final version.

He listens to Obama! Tobad he would not listen to the POEPLE OF MAINE!! Can't wait to see his name in the voting booth again so I can vote for who ever else is running agains him.

His changes that he won't vote for this bad idea is about as much as Baldacci shutting down his failure Dirigo Health. . Mike Michaud will vote just the way Pelosi and Obama tell him too. And as for money coming to Maine When Hell Freezes over, when Pigs Fly come to , Obama will put the screws to Michuad and Maine as soon as he has his vote!

Thank goodness the Mike received "assurances" that our problems with Medicare payments will be solved. Up until now I thought we received nothing for Mikes vote, but we those "assurances" we are saved! I just do not understand that if nearly half the bill is being paid for by cuts on Medicare than how is that going to solve Maine's problem. Are the cuts in Maine going to be less?

agreed zatwoodz....I voted for him in 2006 but I already let him know he lost my vote for 2010 via e-mail. It is exactly this bargaining on the side to get votes that is sending the country down the tubes and increasing pork barrel spending...

Thankfully, Mike Michaud is listening to the people of Maine...we want health care to be reformed!!

The people of the State of Maine, as well as the majority of this nation want healthcare reform. I applaud Mike Michaud for working towards that goal. However, I must agree that the sideshow politics is what leads to rampant spending! Unfortunately you have to play the system the way everyone else does, or the great State of Maine will get shorted! Hopefully the end justifies the means.

Mike went to the whitehouse to solicit a bribe for his vote (like the senator from Louisiana) and discussed the issue over a cold glass of Koolaid. Earth to Mike.. we're not drinking the Koolaid but we're taking notes. I have a vote next November that doesn't have your name on it.

lets see so i guess that means mojority is 38 percent, as that is what the latest polls show support for this health care bill at. the problem is what they are doing is not reform it is killing the plan to force every oen into one plan that they can control. you will have doctors in wasghtion saying what pills you can or cant have, what foods you can or cant eat. Hell look at the news even soem univertiys are trying to make people pass physical education just get a diploma if they have high BMI. Is this the reform you want goverment to be in every part of your life if it is then move becasue over 50 percent that is the majority of people dont want this plan.

In addition to the health care plan, he voted for the Cap and Trade bill which would be devastating to everyone, but especially to the folks in Maine. I know a lot of people who voted for Michuad and are now VERY upset with his voting record. They tell me they would never vote for him again. Michaud is a goner in 2010. He'd better get his fork lift license renewed. I hear the mill calling his name......hopefully it will still be open in 2010.

Truly a sorry situation that an individual elected to an office in Washington is led by nose to vote for higher taxes on his constituents; cause higher medical expenses; to grow government when there is no money; supports to pay for abortions; and pay for illegal alien medical expense. Yet he reflect that he is in bed with Obama and Snowe! Truly a sad situation and just another step into the coffin of our Country as we know it!

I am pro choice, but I see no reason to let an issue like abortion kill a healthcare bill. It is an elective procedure and should not be covered by insurance, period. Plastic surgery isn't covered and that is an elective. Ok, maybe there should be a life of the mother clause or whatever, but that is almost NEVER the case with abortions.

The state of debt we are in: Bush came into office with a surplus, war in Afghanistan drained some money, but i have no problem there. The war in Iraq which was unfounded, unfunded, uncalled for and our objectives never met. How do you expect to not have a massive debt when to date ~$700,000,000,000 has been spent on this year at the time of this post (as given by http://www.costofwar.com/). yeah he brought war spending down. Please take a look at the effect that has had on troop casualties and injuries before you start counting your savings. Is that really what it comes down to for you? none of these things are based in actual fact, you know. You watch too much Hannity. It is not required to put your hand over your heart during the national anthem. I remember this uproar during the elections. He was not the only one on stage not doing so. If you think not wearing jewelry on your lapel makes you unpatriotic, then you need some real therapy. The politicians who brought it up at the time were also not wearing one.

Which enemy countries are you referring to? China and Japan are not our enemies, last time I checked. That is the protocol for meeting leaders in their own countries. I'm sure you can find pictures of Bush doing the same thing, but NOBODY CARED at the time. We do not own the world and greeting leaders according to their custom ( not everyone shakes hands) is what is expected of grown ups in the real world.
