AUGUSTA, Maine — The LePage administration laid out a plan Wednesday to restructure the Department of Health and Human Services into a leaner organization that better serves Maine’s needy.

LePage’s proposed bill would consolidate four DHHS offices into two and reorganize another. It eliminates 91 positions across various levels and creates 44 jobs for a net loss of 47 positions.

Several of the jobs under the ax are vacant.

DHHS employees were notified of the planned cuts earlier Wednesday.

In the works for several months, the restructuring bill is not designed to address the department’s current budget gap, according to DHHS Commissioner Mary Mayhew. The legislation also predates a computer error that came to light last week which led the state’s Medicaid program to continue paying medical bills for up to 19,000 beneficiaries after they became ineligible for the program, she said.

“We have been focused on evaluating this department since last summer in terms of our vision for this level of integration,” Mayhew said.

The bill, expected to be submitted this week, will require approval by the Legislature.

The proposal seeks to merge the Offices of Substance Abuse and Adult Mental Health Services and combine the Offices of Elder Services and Cognitive and Physical Disabilities Services.

The Office of Child and Family Services will reorganize and link together its four major service areas, including child welfare and behavioral health.

The Office of MaineCare Services and the Office of Family Independence, which oversees food stamp and child support programs, are not targeted under the restructuring.

Officials still are nailing down the reorganization’s financial effect, but early predictions are for modest savings of around $500,000, Mayhew said.

The savings might be used to draw down a wait list for services for adults with developmental disabilities, she said.

Lawmakers continue to grapple with a roughly $85 million shortfall at DHHS looming in the next fiscal year. Late last month, Gov. Paul LePage signed into law a measure that closed a $120 million budget gap this year, allowing DHHS to keep paying its bills through June 30.

Meanwhile, the legislative session is quickly approaching its April wrap-up.

The restructuring legislation is expected to be printed “any day now,” followed by a public hearing within several more days, Mayhew said.

The goals of the shakeup are to shift resources from middle management to front line services, and to make DHHS’ web of various services easier to navigate, according to Bonnie Smith, deputy commissioner of programs.

DHHS administers a range of health and social services including mental health programs, foster care and oversight of public drinking water. It has offices from Sanford to Fort Kent and employs roughly 3,600 people.

Under the restructuring, contracts will be consolidated to cut down on administrative costs. The overhaul also would help DHHS better serve children in the system who fall through the cracks after reaching adulthood, Smith said.

Hospitals, clinics and other health care providers should benefit from better communications within the department, according to Mayhew.

“We would clearly hope that the administrative burden, at a minimum, on providers and agencies could be reduced as a result of this,” she said.

If approved by the Legislature, the restructuring would mark the first substantial overhaul of DHHS since it formed in 2004 through a merger of the Department of Human Services and the Department of Behavioral and Developmental Services. That consolidation, too, was designed to improve efficiency.

“Back in 2004 when the departments merged to create the Department of Health and Human Services, the result was that while much of the work merged, some of the things still didn’t come together and much of our services and our programs were designed in silos,” Smith said.

While DHHS officials said the restructuring bill was unrelated to last week’s Medicaid computer problems, Mayhew has cited poor communications within the department as a factor in the bad payments.

Rep. Mark Eves of North Berwick, the lead House Democrat on the Legislature’s Health and Human Services Committee, said he sees a direct link between the computer problems and the loss of jobs outlined in the restructuring proposal.

LePage has swept a host of high-ranking DHHS managers from their positions since taking office, leading to a “brain drain” within the department that contributed to the erroneous payments and a budget proposal based more on ideology than sound information, Eves said.

“We will keep an open mind” about the proposed reorganization, he said. “We do have a lot of questions.”

The Health and Human Services Committee was briefed Wednesday on the planned restructuring.

Republican committee co-chair Rep. Meredith Strang Burgess of Cumberland said she’s optimistic the bill will reduce duplication and finally tie up loose ends from the 2004 merger.

She credited the proposal for shifting the focus from management to people who use DHHS services.

“Anything that helps the people of Maine, I’m all ears,” she said.

        This Governor tells us we can’t afford it and adjustments and cuts must be made.

  2. Really they ought to target the very excessive salaries that the non-profit sector that is funded almost exclusively by MaineCare pays it top execs.

     One here here in Herman, a small non-profit with less then 350 employees pays its CEO over $120 k a year, plus bonus benefits that only the CEO gets.

    That is outrageous for the Bangor area. I even heard this CEO say “I did not prepare for my retirement so XXX needs to pay, and then got a bonus retirement plan exclusively for them.

    These people have the nerve to ask their employees making 8 or 9 an hour to donate to the company, while paying these excessive salaries.

    Lets pass a law limiting salaries for non-profits  funded by taxpayer money, they should not be getting rich in a non-profit.

Do you have any idea what middle level mangers make in the private medical care field?  I’m not talking about physicians and nurses.  I’m taking about financial and billing officers, compliance officers, and the like?  It would suprise you.  A lady I know down here was a midlevel finance officer at a small local hospital—salary 170,000—she left to take a better paying position with less work in the midwest.  If you want to keep costs low don’t privatize it—that gives the companies who are awarded these contracts a license to steal

Mainecare rates are standard currently across Maine for services billed, and have been cut in areas up to 25% in the past few years.  Which means the company in Bangor or Portland gets the same rate as the company doing it in Houlton or Fort Kent.  The only way to have that type of salary is to cut other things in their non-profit agency, such as pay for front line workers, or crappier benefits, or horrible staffing patterns.  The money in a non-profit has to be balanced.   And if you actually use a statewide tool for income levels of CEO’s of non-profits, 51, 204 is shown as Executive Director of non-profit agencies as a median. I think that was on payscale.com.

I had wonderful benefits but I was at executive level, no cars, wasn’t that high.  My pension will be starting soon and I got six week vacation, 10 sick days, 2 personal days and my birthday off, PAID.  Thank you to all, I just wish people would understand the scam they are running.  The ins companies run the hospitals now.

Actually, to get this pay by the board of directors, they first pulled CEO salaries in the Bangor area, not enough, included bigger corporations because “a non profit is much more comlicated” not enough.           1. You are mistaken, they do not fund raise. Is very creative in finding “loopholes” though, like the one that resulted in a 1.5 mill payback for fraudulent billing. I fully feel that supporting people with disablities is a good service, but there are other agencies that are not profiteering from it.

The days of non-profit directors making six figures are coming to a rapid end, as well they should be.  In this budget environment, non-profit organizations should not be paying directors six figure salaries. Those that are, may benefit from close investigation by their board of directors. And that is coming from a liberal. 

      Despite that, this bull-nosed attack on DHHS (which happens to be the biggest line on the State’s budget) is ignorant, impulsive and irresponsible.  I get that Penguin hates people who rely on State Aid (including, as we learned this week, the people in the Katahdin region).  But his reliance on Junior High School Playground bully tactics is concerning to me.  Most of us realized by the 9th grade that those kids were all hot air, and had pretty sad stories.  This guy seems to think it’s a leadership philosophy. 

      You can’t initiate major policy changes without support.  The move Penguin is making here is not unlike the kid who declares himself king of the hill. Then the whole recess population joins forces, turns on him and send him off to find a new hill.

You get what you pay for.  The Feds constantly blotch Information Technology projects at great cost to the tax payer because fed IT salaries are not competitive.  Maine tends to have IT problems (like the Maine Care payment problem) for the same reason.  I don’t know the nature of the not for profit cited, but $120K for managing 300+ staff doesn’t strike me as unreasonable.

  11. I think it makes sense to put mental health and physical disabilities together. A disability or challenge is that whether it is physical or mental. Same goes with abuse.

    I think child and family services is a joke. Keep cutting. All they do is contract to another company that does half assessments. They do not help the needy or the one who asked for help, only hurt them and listen to and encourage the other party to spread rumors, lies, and exaggerations.

  15. So, does this mean clinics and heath care providers are NOT going to treat these patients as piece work billing the state 135.00 per visit. And treat all complaints at one visit?   or is it going to be business as usual bill 135.00 per visit and have the patient keep coming back 2 , 3 times a week?    This is where a lot of the money went. 

    1. oronowoman, you have to frame your argument a little better. We don’t know what to do with it…what’s your perspective etc. I was in healthcare for 10 years. It’s all billable…often to the detriment of the consumer.

      1. mainecare patients are treated as piece work..  these clinics that see mainecare patients get a bonus  for rushing them in and out as fast as they can..  these patients come back to the same clinic  sometimes  3 times a week.  so the clinics can bill another 135 .00 per visit 

        1. all the docs I’ve ever known have always rushed me out, and for most of my life I have paid out of pocket, even during the short period I had to use Mainecare…and when I had my own insurance.  Last doc I saw was a neurologist for my migraines. 15 minutes…$300…all of the docs that I have taken my mother to same thing…and she’s on medicare. Anecdotal, but I don’t think you will find too many docs these days actually spending time with their patients, regardless of  how the patient is paying for the appointment.

That’s true, my mother was getting medicare, and she had two private insurers, Aetna, and BC + BS.  she went to the doctor once a month for the last 15 years (since she had a pacemaker installed) and each visit was the same. she went in to the office, they pricked her finger, and tested the blood with a meter at the office.  The procedure took fewer than 5 minutes and we were billed at least $140.00 per visit. I figure that over that time we gave that office over $25,000  The great part was we hardly EVER saw the doctor, a physician’s assistant did this test. AND she told me I could probably do it at home if I had the meter.

            …..and they wonder why Medicare is running out of money.

  19. Here we go again! Every new administration decides to reorganize to make the department “leaner & more efficient” but they never bother to learn how to do it so it will work. They get recommendations from adviser’s, (aka, friends & contributors who need jobs), and business people who are hoping to make a buck with a newly privatized contract, then issue commands that do nothing more then change letterhead! When I worked for DHS in the 80’s & 90’s we went through this and many bills were  put through the legislature with no thought on how they would be enacted…many were never implemented because the law may have changed but no procedures were put in place to make the laws work. It was a joke, as most of these reorganizations are. Just wait a few years and the rules will be changed again to fit the new administration’s political BS. If they want to make changes that work, they need to listen to the people who are working with the problem every day, NOT to political appointees and elected officials who are always running for office.

So he wants to eliminate many currently “frozen” positions, to save money, and bring more work to the front lines of DHHS offices, where he eliminated many vacant positions because they were frozen and unable to fill them, so who at the front lines in the DHHS offices is supposed to serve the public secter and push the papers in the office. I think they should eliminate half of the over paid and under worked upper management and hire for the vacant positions of course these are the positions that are under paid and over worked. But at least they are the ones that make things happen for people in the community.

  24.  It’s becoming more obvious all the time that LaPudge doesn’t have much under control, he doesn’t have his mouth under control, he doesn’t have his appointees under control, he doesn’t have DHHS under control, he doesn’t have the legislative process under control.  He’s been governing by chance in accordance with his tea bag philosophy, and as has been the case in Washington, other states and Maine, the philosophy is weak at best. 

    1. The Baldacci admins DHS is being purged finally.I am sure the dems spinning this into blaming Mr lepage as he uncovers past waste and fraud. Does not bother the governor as long as he gets it fixed.

  26. It seems like most of the news that comes out of DHHS concerns bungles and fumbles.  And now an effort to “restructure” the department is underway.  Why does this not inspire confidence?  (I think Mary Mayhew must hate getting up in the morning.)

  31. 47 Jobs cut saves only 500,000 dollars?  Those jobs all get paid less than minimum wage all of sudden, or are the jobs you are adding costing us much more?  Do the math, $10,600 each for those 47 jobs lost…..and they wonder why no one know where any of the money is!

The managment of hospitals are way over paid which is why we need ACA, I was at a mgmnt level and I know how it works, new vehicles, trips, conventions and the country club.  Thank you to all of you that paid premiums.  The ins companies treat us well on your money.  This bagger governor is just loon who will continue the destruction of ME.  ME alreaedy has a D in education, makes you proud doesnt’ it? 

    The whole think about computer glitch is straight up BS, that is human error, not brain drain.  I can assure you they aren’t programming.  The first system you bought was junk too but cheap.  As long as his retirement and his daughters are exempted from the taxes he is putting on, that’s what’s important.  Watch as retirees leave the state and your taxes go up.  We can live anywhere and I’m not supporting that dummy and his policies.

So true what you are saying, Insurance companies give a LOT of money to the politicicans.  The whole thing is political.  You’re right with the computer glitch,  they never did audits.  They just started doing audits recently when they were looking into Mainecare.  Enjoy your country club!  

      1. I was outsourced with no Golden Parachute like the CEO, I was a technical prof. They give a lot to the hospital, MDs and so on, it’s ridiculous. That’s why it costs so much. It is illegal to refuse care so the ER becomes their MD and the cost gets passed on. :(
