BREWER — The Maine chapter of the Society for Social Work Leadership in Health Care recently presented social worker Jaime Boyington Rogers, Eastern Maine HomeCare Community Care Team coordinator, with the Social Work Leader of the Year. The award is given to one social work leader annually who exhibits exceptional talent and professionalism in the field of social work. Rogers has been a social worker for more than 20 years, and has worked across the continuum of healthcare services, including inpatient-outpatient mental health, integrated behavioral health settings in school based health care, nursing homes and now in community care teams. In her role as community care team coordinator at Eastern Maine HomeCare, Rogers is involved in redesigning the Eastern Maine HomeCare health care delivery system in ways that meet individual patient needs in the most cost effective way. Nurse Lisa Harvey-McPherson, president and CEO of Eastern Maine HomeCare said, “Jaime is a leader who goes above and beyond, practicing ethically and working collaboratively with her colleagues to make a positive difference in the lives of patients and consumers. She is a positive role model for all social workers and brings an enthusiasm and creativity to her practice.” Currently, Rogers and her community care teams provide support for the most complex, high risk, high need and-or high cost patients being cared for by 43 EMHS and other primary care practices. As coordinator for the program, she manages teams throughout Maine, including Greenville, Bangor, Waterville, Portland, and northern Penobscot, and most of Aroostook County. In this role, she has implemented tracking tools and initiatives to help support primary care practice teams improve the overall healthcare of literally thousands of patients within the system. Rogers was recognized during the Maine Hospital Association annual meeting in June.
